Rear wheel axle

I have a 26 inch fat tire ebike that I have had since Aug 2025.Lately I have been able to commute and ride trouble free. The other day I had a rear flat tire. I promptly repaired the flat but ever since I did this, now the wheel tries to eject itself from the rear dropouts. I have tightened and re tightened. these axle nuts while it is upside down. As soon as I flip it over and get ready to ride, the wheel comes loose. It doesn't come all the way off because of the torque arms but it makes the wheel slip into a position where it rubs against the frame.I noticed that the axle nut is not gripping the axle on one side

Could the no-turn washer be upside down? The washers are likely on the wrong order.

Not necessarily correct, as you can put the tabbed washer on the inside and regular washer anywhere it fits, but the tabbed washer usually faces up

You want the axle to sit in the bottom of the slots on both sides too,
