I just checked out my wheel with velofix and it's definitely the hub. I spoke with Rad bikes and they are going to send me a new motor. Bummer, I have to wait another week to ride my bike. Here is a video of the hub noise:

If I spin the wheel very slowly, you can hear clicks. I can made it click one at a time when spinning slowly. It sounds like the wheel from Wheel of fortune.

Its not the derailer, it is shifting correctly. Its different than the freewheel as well, it clicks in both directions and clicks even if I am spinning the wheel with my hand off the ground. I put a video in the last post here...

Thanks so much @Dewey for the details, this is great information! I looked at the Qibbel but it said the rack pipe diameter needs to be smaller than 16mm and the radwagon is 12. I bought the Polisport so hope it fits, it didn't mention the max diameter and the installation video looks like it...
