My best guess is that it became fatigued over time - I have just under 10k km on the bike. I put a lot of torque on the pedals getting out of a tight spot and I think that did it. While that burst caused the chain to jump off, I did not view it as being outside the bounds of normal operation.
I agree. The bike is currently at the shop where I bought it and their first comment when I showed them the bolt was something along the lines of "I am not sure where we would get that".
