Random clunk in front wheel

I'm refurbing a used and abused Eahora x3. When riding I've noticed a random "clunk" from the front wheel assembly. I checked that the disc rotor and pads are lined up straight so no interference there, and no wobble or looseness on the axle that I can feel. What else might cause something like that?
 
The front wheel was torqued on so tightly I almost needed a cheater on the wrench to break it loose. Cast wheels with two bearing Inserts, so nothing fancy (or really serviceable). Bearings not in great shape, but what you'd expect on a bike of this "quality". I tried to get some lube in the bearings and reassembled at more correct torque values. No clunking anymore (was probably due to the gorilla level torque on the axle nuts originally), so now just the cheap bearing noise. Good enough!
